1099R's for Hourly and SERP Plans for Tax Year 2019:
Please note the 2019 1099R tax forms were mailed between January 12th & 15th, 2020. If you require a duplicate or have any issues pertaining to your 1099R, please contact Northern Trust at 312-557-9700. The Voice Response allows you to auto request a duplicate via mail, or you can request to speak with a representative if there is an address change or other.

Salaried Pension Plan:

• If you rolled your ULA salaried Pension into your 401(k) account, you will see it in the Rollover Balance, which can be accessed from the ULA 401(k) Savings Plan information above.
• If you opted to convert your ULA Salaried Pension benefit to an annuity and have questions, please contact Athene at 1-877-813-4240 or www.lifeatworkportal.com/Athene
